Providing outreach support to vulnerable service users in West London. You will keywork, refer service users to external services as well as work within multi- disciplinary teams. You will be responsible for updating case notes and attending internal and external meetings and training. You will work with clients to develop independent living skills and find education,employment or training to meet national minimum standards. Knowledge of housing benefits, as well as Supporting People Legislation is a plus in this role but you must have one years experience working in a social housing setting.

The hours are full time and the post is to cover recruitment for permanent staff. Candidates need to have relevant experience of working in Child Protection and Social Services and experience of working for a tier 3 drug agency, and of care planning, organising prescribing services and keyworking clients directly. Candidates must have worked with clients in relation to their drug related issues, as opposed to working with clients who have taken drugs.
